All Saints HealthCare is dedicated to developing alternative methods of care for patients
with complex medical needs. This dedication stems from the belief
that medically complex patients have unique care needs that cannot always be met in a traditional acute care unit.
These needs include intensive therapy,
emotional support and an individualized plan developed by a comprehensive and caring medical team. Our subacute
care program provides a medical team
consisting of a Physician, Dietician, Respiratory Therapist, Physical Therapist, Activities Coordinator, and other
specialized personnel. Family members are
encouraged to attend care plan meetings to learn first-hand how their loved one is progressing. Our goal is to
maximize improvement for each patient and
to provide support for the entire family.
All Saints HealthCare has established a policy of having family support groups at each
of our subacute units. Our staff is here to help the families and friends
of our residents with their problems and concerns, just as we are here for the residents. As a friend or family
member of a new resident to the subacute unit,
family support group meetings can offer an opportunity to learn about how the unit is structured - who does what
- and how to get questions answered. More
importantly, you will learn how you fit into the care of your loved one. We will help you to understand the importance
of your involvement in improving the
quality of life for your loved one.
Speakers from the community are often invited in to discuss issues such as Medi-Cal, conservatorships,
medical aspects of subacute care and other issues
of interest to the families. More than anything else, the Family Support Group gives each friend and family member
the feeling that he or she is not alone in
facing and dealing with the complex and sometimes painful issues of having a loved one in need of long-term care.
